Following your referral to The Beacon Service a member of The Beacon health care team will contact you. We can arrange to see you either at home, at The Beacon Centre or at one of the community hospitals.
It is usual for us to speak with your GP or District Nurse and sometimes your Consultant to help us to understand more fully about your situation. Your needs will be individually assessed. During this assessment you will have the opportunity to talk with a member of The Beacon team about issues of importance to you and any concerns or problems relating to your illness. We will discuss with you about the range of care and support available from The Beacon Service and how this may help with your individual needs. |
